Parental Permission Form

As part of the PDHPE programme, our course unit overview will cover a discussion of body changes, both male and female, as well as the secondary cycle. Some sexual awareness topics will be covered in a way that relates to puberty, the changing teenage body and sense of self.

As part of the lesson plan, we would like for parents to be aware and actively involved in starting communication with their child regarding the changes associated with puberty and adolescence. The lessons will include specific information relating to menstrual products, and all students will be offered a range of product samples to assist in their understanding of the menstrual process.

Download the permission form here and make copies of them to distribute to your students, for them to take home and have it signed by their parents.

The form requests their parent’s consent to their child receiving samples of Ultrathin pads, liners and/or tampons as part of the PDHPE unit – My Changing Body.
